Heritage Health seeks to provide access to its community by offering expanded hours for acute care needs, including weekends, to reduce utilization of urgent care and emergency rooms. As a Provider working in a walk-in / Rapid Care clinic environment, you will provide comprehensive, patient-centered care to individuals and families with acute and chronic needs requiring immediate access to care. This role will not carry a patient panel. You will deliver high-quality medical services, including health assessments, diagnosis, and treatment of acute and chronic conditions, preventive care, and patient education for our diverse patient population, including pediatrics. By ensuring accessible, equitable, and culturally responsive care, you will support our mission to reduce health disparities and improve the overall health and well-being of the communities we serve.

Salary : NP / PA $129,422.00
