Schedule :
Overnight 7on / 7off Monday through Monday (7pm-7am), includes a $6 / hr shift differential

Evaluate physician medication orders for drug appropriateness, correct dosage, potential drug interactions, and proper route of administration.Provide clinical recommendations to physicians and facility staff regarding medication therapy changes and optimizations.Maintain up-to-date patient / resident profiles, including medical history, lab results, and current medications (prescription, OTC, and home remedies).Ensure laboratory tests required for monitoring high-risk medications are appropriately ordered, reviewed, and communicated to the healthcare team.Collaborate with physicians, nurses, and other healthcare professionals to provide clinical and technical support.Document all communications and clinical interventions in the patient’s medical record, including interactions with families and caregivers.Implement and enforce policies and procedures for the safe procurement, storage, administration, and disposal of medications and biologicals.Ensure compliance with all state and federal regulatory requirements related to pharmacy practice and medication management.Report medication-related errors and adverse drug events to the Pharmacy Manager as required.Educate and support facility / agency staff on safe and effective medication handling procedures.Perform other pharmacy-related tasks and responsibilities as assigned.Qualifications
Qualifications
Required :
Bachelor’s Degree in Pharmacy, Pharm D preferredOne year experience in long term careLicense / Certification(s) : current and valid pharmacist license in the state of practiceKnowledge of drug therapy guidelines, medication safety practices, and regulatory standards (CMS, DEA, FDA).Proficiency in electronic medical records and pharmacy information systemsStrong clinical judgement, and effective written and verbal communicationPosition will be posted for a minimum of 7 business day
