Program Therapist -PRN

Responsibilities

Salt Lake Behavioral Health is seeking a Per Diem Program Therapist (Social Worker / (A)CMHC) for various units.

Schedule : Per Diem

Job Description :

As part of the social services team, this is a position that will complete biopsychosocial assessments, develop treatment plans, provide individual / group therapy and assist in discharge planning. This position will work collaboratively with the treatment team, families, community resources, and other healthcare providers.

Job Essentials :

• Prioritize daily tasks including new admissions and discharging patients along with seeing patients on given case load.
• Participate daily in a multi-disciplinary treatment team meeting.
• Conducts psychosocial assessments, ASAM's and health disparities form for new admissions.
• Educate, communicate and coordinate discharge plans with patient's families and support systems.
• Provide daily individual therapy along with psychoeducational groups.
• Complete discharge planning including crisis safety plans, family meetings and discharge summaries.
• Complete documentation in a timely manner according to the given standards of the hospital.
