Job Description

Job Title : Registered Nurse

Shift : Day 5x8-Hour (08 : 00 - 17 : 00)

Experience : Minimum 1 full year of Adult Home Health or Hospice experience

License : Must have license in hand

Certifications : CPR, BLS

Must-Have :

• Proficiency in HomeCare HomeBase (HCHB) and must have used it within the last 6 months, or be willing to complete a retraining course.
• Proficiency in OASIS for RNs.
• Reliable transportation, current driver's license, and liability insurance.
• Ability to travel up to 75 miles from the home clinic.
• Willingness to accept all patient assignments regardless of location.
• Completion of required documentation within 24 hours of patient appointments.
• Availability for on-call rotations, including 1 weekend per month.
