CASE MANAGER - CASE MANAGEMENT DEPARTMENT

Full-Time | Jamaica Hospital Medical Center | Queens, NY

Jamaica Hospital Medical Center , a leading healthcare facility in Queens and member of the MediSys Health Network, is seeking a dedicated Case Manager (RN) to join our Case Management Department.

As the first and only Comprehensive Stroke Center in Queens, we continue to expand our services, including a modernized Emergency Department and affiliation with Memorial Sloan Kettering Cancer Center.

Key Responsibilities

  • Conduct comprehensive assessments of medical, psychological, and social needs.
  • Develop and implement individualized care plans with patients, families, and interdisciplinary teams.
  • Coordinate patient discharges, ensuring safe transitions to home or other care settings.
  • Document care in the EMR accurately for compliance and quality reporting.
  • Collaborate with physicians, nurses, social workers, rehab, pharmacy, and other professionals.
  • Educate patients and families on medications, follow-ups, and community resources.
  • Manage high-risk and complex patients, proactively removing barriers and preventing readmissions.
  • Advocate for patients and coordinate access to financial, social, and community resources.
  • Ensure compliance with Joint Commission standards, CMS regulations, and hospital policies.
  • Address unexpected challenges using problem-solving and crisis intervention skills.

Requirements

  • Valid NY State RN license
  • Bachelor's degree in Nursing; case management experience preferred
  • Strong organizational, problem-solving, and communication skills
  • Ability to work independently and collaboratively in a fast-paced environment
  • Compassionate, patient-centered approach
  • Familiarity with EMR systems, medical terminology, and healthcare documentation
