Description : Overview : The Supportive Housing Case Manager provides direct support to chronically homeless adults in a permanent supportive housing (PSH) program. This role helps residents achieve stability, independence, and well-being by connecting them to community resources and coordinating individualized services. The ideal candidate MUST have a background working with the chronically homeless population, including individuals with Serious Mental Illness (SMI) and co-occurring (dual) diagnoses. Key Responsibilities : Conduct client intake, assessments, and individualized service planning. Assist clients with housing search, placement, and retention support. Provide ongoing case management, including resource referrals and progress monitoring. Respond to housing crises and coordinate with emergency or support services. Collaborate with community partners, including healthcare and behavioral health providers. Advocate for client needs with landlords, agencies, and service systems. Maintain accurate records and required documentation. Qualifications : Master’s degree in Social Work preferred; Bachelor’s in Human Services or related field required. Minimum five (5) years’ experience working with homeless or high-needs populations. Knowledge of community resources, housing systems, and case management best practices. Strong communication, organizational, and problem-solving skills.. Experience with diverse and marginalized populations. Schedule : Full-Time Monday through Friday, 9AM TO 5PM.
