This Nurse Practitioner position is based in a Skilled Nursing Facility (SNF) setting, serving as a clinical leader to oversee short- and long-term residents.

Responsibilities include admissions, discharges, monthly stability checks, evaluations, advanced care planning, assessments, and prescribing treatments.
