Float Nurse Practitioner

Community Care Physicians is seeking a float Nurse Practitioner. The float NP will travel across the region where CCP practices are located to help with provider vacancies and to ensure continuity of care for our patients.

The float Nurse Practitioner will have a rotating schedule and will assist practices in providing same day care, transitions of care, surgical clearance exams and other forms of visits based on the practice need.

Working schedule is flexible, based on candidate preference can be a 4 day or 5 day work week.

Responsibilities :

• Collaborate with physicians in managing acute and long-term medical needs of patients.
• Assess patient status by obtaining health history.
• Order / perform laboratory diagnostic and other screening tests.
• Treat illnesses, give injections, suture wounds.
• Monitor therapies and provide continuity of care.
• Counsel patient / family on preventive health care.
• Participate in peer reviews, chart reviews, staff education, clinical guideline development and other continuing education.

Compensation : $100,000 - $150,000 annually
