Registered Nurse (RN) – Home Health –San Fernando Valley and Surrounding areas

Pay : $50 to $80 per hour (Depending on Experience)

Schedule : Full-Time, Part-Time, and Per-Diem Positions Available

Help patients reduce risks of falls and avoid unnecessary hospital readmissions

Provide education on chronic conditions and proper disease management

Perform full medication reviews to ensure safe dosage, frequency, and minimize side effects

Deliver advanced wound care services for safe and comfortable healing at home

Help patients avoid costly and time-consuming office visits by receiving expert care at home

Your Responsibilities

Conduct in-home patient assessments and develop individualized care plans with physicians

Deliver direct skilled nursing care, education, and emotional support to patients and families

Complete comprehensive medication reviews and provide guidance for safe medication management

Perform wound care, mobility support, and post-surgical recovery assistance

Educate patients on chronic disease management, preventive care, and self-care strategies

Communicate closely with physicians and care teams to coordinate ongoing care

Document patient care accurately and in a timely manner

Promote independence and safety within the home environment

Qualifications

Drivers license

Current, active RN license in California for a minimum of one year

At least one year of professional nursing experience (home health experience strongly preferred)

Public Health Nurse (PHN) certification preferred

Strong clinical skills, organization, and communication abilities

Ability to work independently in the home setting
