Bachelor of Science in Nursing (BSN) degree, active registered nurse (RN) license and minimum 1+ years RN experience required. Applicants who do not meet these qualifications will not be considered.
A Hospice Registered Nurse (RN) is needed to provide compassionate, patient-centered end-of-life care in home and residential settings. This full-time, 13-week contract role involves managing hospice care plans, symptom management, and emotional support while collaborating with an interdisciplinary team to ensure patient comfort and dignity.
Location
This position is based in Scottdale, Pennsylvania, an area known for its friendly community and serene surroundings. Nurses will travel daily to patient homes, assisted living facilities, and nursing homes within the assigned service areaa rewarding role for those who enjoy variety and direct patient interaction.
Key Responsibilities
- Conduct comprehensive nursing assessments and develop individualized hospice care plans
- Provide skilled nursing care focused on symptom management and patient comfort
- Administer medications and treatments per physician orders and hospice protocols
- Monitor patient condition changes and communicate promptly to the care team
- Educate patients, families, and caregivers on disease progression and support strategies
- Collaborate with social workers, chaplains, aides, and volunteers to address patient needs
- Complete timely and accurate documentation using Home Care Home Base (HCHB)
- Participate in case conferences and quality improvement initiatives
- Ensure compliance with hospice policies and regulatory standards
Qualifications
