As a vital member of our healthcare team, you will provide essential evaluation and physical therapy treatment in the comfort of patients' homes. Your role will ensure compliance with all agency standards and regulatory requirements in Maryland, Virginia, or the District of Columbia.
Key Responsibilities :
- Assess and document patient and family responses to therapy interventions. Evaluate and track progress towards goals, adapting treatment plans as necessary.
- Create personalized teaching plans for patients and families based on their knowledge levels and prescribed treatments, ensuring effective communication at all stages.
- Contribute to departmental goals while adhering to policies, procedures, and safety standards, completing required training and compliance tasks.
- Implement and adapt comprehensive treatment plans in collaboration with patients, families, physicians, and healthcare team members.
- Conduct thorough evaluations using various specialized assessments and tools, ensuring precision in all measurements.
- Manage patient assignments efficiently, seeking support from senior staff when addressing complex patient needs.
- Engage in regular case conferences and collaborative goal-setting sessions, including patients and their families.
- Build positive relationships across departments and actively participate in quality improvement initiatives.
- Identify patient rehabilitation needs and recommend necessary adaptive equipment or modifications, facilitating optimal care and safety.
- Perform essential physical therapy procedures and modalities to promote recovery and enhance quality of life.
Qualifications :
Bachelor's degree in Physical Therapy from an accredited institution.1-2 years of clinical experience in physical therapy.Current Physical Therapist License to practice in the relevant states and a valid driver's license with a dependable vehicle.CPR certification (including BLS and NRP) within 90 days of hire.Additional certifications may be required based on department needs.Skills and Attributes :
