Position Title : Pharmacist

Department : Pharmacy

Supervisor : Pharmacy Director

Status / Hours Per Week : Non-Exempt / Full-Time

Primary Function :

Under the supervision of the Pharmacy Director, the Pharmacist plays a vital role in promoting health outcomes and enhancing patient care within the community. The Pharmacist dispenses medications prescribed by healthcare providers and delivers essential information about their use, safety, and efficacy. Collaborating closely with patients and practitioners, the Pharmacist advises on medication selection, dosages, interactions, and side effects while addressing patient concerns and fostering a supportive environment

Duties & Responsibilities :

Patient Communication

Provide clear and compassionate communication with patients, caregivers, and healthcare providers, addressing questions related to drug interactions, side effects, dosage, and proper storage of medications.

Patient Care Collaboration

Actively engage in patient treatment plans, serving as a key liaison between healthcare providers and patients to promote comprehensive care and health equity.

Medication Management

Perform thorough drug utilization reviews and assessments to ensure the appropriateness of therapy and adherence to evidence-based guidelines.

Prescription Review

Verify prescriptions for accuracy and assess the need for specific formulations, ensuring safe and effective medication use.

Compounding Oversight

Supervise pharmacy technicians in the compounding, mixing, packaging, and labeling of pharmaceuticals, ensuring compliance with safety standards.

Standardized Procedures

Implement and maintain standardized processes for filling prescriptions, ensuring consistent and efficient service delivery.

Regulatory Compliance

Provide oversight to pharmacy staff on compliance with state and federal regulations, fostering a culture of safety and ethical practice.

Quality Improvement

Guide pharmacy staff in adhering to departmental policies related to customer service and patient safety, contributing to continuous quality improvement initiatives.

Complaint Resolution

Address patient complaints and returns per UCHC policy, coordinating effective resolutions with pharmacy staff.

Operational Efficiency

Meet deadlines for prescription fulfillment, operational reporting, billing, and other time-sensitive activities.

Inventory Management

Monitor inventory levels to prevent overstock and waste through effective stock rotation and management.

Continuing Education

Participate in ongoing education and attend required professional development programs to enhance clinical skills and knowledge.

Immunization Delivery

Administer immunizations as required, maintaining current certifications in immunization delivery and CPR.

Health Initiatives

Support and participate in community health initiatives aimed at improving access to pharmaceutical care for community residents.

Qualifications

Minimum Basic Knowledge :

  • Graduate of an accredited school of pharmacy; PGY1 / 2 residency completion is strongly desired.
  • Licensed to practice pharmacy in the state(s) where services are provided, complying with state CEU requirements.
  • Certifications in disease state management are a plus.

Experience & Qualifications :

  • Proven supervisory skills with the ability to cultivate a collaborative, team-oriented environment.
  • Strong ethical judgment and business practices in interactions with employees, customers, and partners.
  • Exceptional written and oral communication skills, with the ability to lead and mentor a diverse team.
  • Previous experience in community pharmacy or healthcare settings preferred.
  • Proficiency with pharmacy computer systems, Windows, and MS Office.
  • Fluency in Spanish and / or Portuguese is highly desirable.
  • Current certifications in APHA immunization delivery and an active CPR certificate are required.
  • Independent Action :

    Independent decision making as required by duties and responsibilities of the position.

    Supervisory Responsibility :

    Supervise pharmacy support staff members.

    Define Access Level to PHI : Level 4 : Authorized to access full health information. UCC position and job responsibilities involve the provision of patient care and working as a clinical team member. Staff in this category level, although allowed full access, should only access the necessary information for each respective treatment encounter / circumstance.
