Key Functions / Responsibilities

Builds relationships with assigned members to understand their concerns, needs, and works toward collaborative goal setting.

Qualifications and Education Requirements

Knowledge of community health resources and social determinants of health

High school diploma or GED required. Associate's degree in human services or another correlated field preferred.
