Position Overview

We are seeking a dedicated Physical Therapist (PT) for a 13-week contract position within a skilled nursing and rehabilitation environment. This role is ideal for a PT experienced in geriatric care, functional mobility improvement, and post-acute rehabilitation. The PT will deliver high-quality therapeutic services to support functional recovery and enhance patient independence.

Key Responsibilities

• Perform comprehensive physical therapy evaluations to determine mobility, strength, and functional needs.
• Develop and implement individualized treatment plans based on evidence-based practice.
• Provide therapeutic interventions focused on strength, balance, gait, and functional mobility.
• Document evaluations, daily notes, and progress reports in alignment with facility and regulatory guidelines.
• Collaborate with an interdisciplinary care team to ensure coordinated, patient-centered care.
• Educate patients, caregivers, and facility staff regarding exercises, safety, and functional strategies.
• Adapt treatment approaches to meet diverse patient needs within a fast-paced post-acute setting.
