Hiring Package :

• $250,000 Base Salary Year 1
• Medical, Dental and Vision available
• STD and LTD available
• Up to $10,000 relocation allowance
• 2,500 CME allowance annually
• 1 : 4 average weekend call
• Join a provider group with m ore than 40 clinic location s all over Arizona
• Join more than 60 Nephrologists
• Advance Practice Providers
• Southwest Kidney Institute is a founding member of Renal Care Organization – National Platform representing approx. 250 providers and nearly 300k CKD patients
