State Licensed Clinical Liaison

A company is looking for a Clinical Liaison. Key Responsibilities Manage the immediate referral response process to meet performance goals Conduct clinical assessments and evaluate prospective resident referrals Communicate determinations effectively with referral partners and the community Required Qualifications, Training, and Education Active and in good standing Registered Nurse (RN) or Licensed Practical Nurse (LPN) license Minimum of two years of experience in a similar role in a LTC / SNF / AL / MC setting Experience in clinical liaison and admission coordination Intermediate to advanced computer skills, including experience with EMR or HER Ability to make independent decisions and handle confidential information
