Job Description :

The Licensed Practical Nurse Clinical Coordinator (LPN CC) or Licensed Vocational Nurse Clinical Coordinator (LVN CC) supports Clinical Team Managers in planning and implementing quality care by advancing key workflows throughout the patient journey. This position monitors the accuracy and documentation of Homecare Homebase (HCHB) inputs and follows-up to ensure workflow requirements are satisfied. The LPN / LVN CC also acts as a key intermediary between the patient, the CTM, the patient's physician or medical provider, and other facility staff overseeing patient care.
