A company is looking for a Health Coach. Key Responsibilities : Establish health improvement plans based on member specifications Support customers in achieving personalized health goals through behavior changes Communicate results of reviews and ensure accurate documentation of clinical decisions Required Qualifications : Associate's degree required LPN / LVN with a valid, current license issued by the state of residence Ability to complete assigned work in a timely and accurate manner Ability to work independently Experience in customer service is preferred
