Summary : The Triage Clinical Nurse is responsible for assessing patient symptoms over the phone or through digital communication systems, determining the urgency of care, and providing appropriate clinical advice or care coordination. This role serves as a frontline responder in managing patient needs, ensuring timely medical attention, and directing patients to the appropriate level of care. The position also includes assisting with patient care and nursing only clinical visits. The nurse must provide quick and precise assessment of patients along with providing caring relationships with clinical patients and families.
ESSENTIAL DUTIES and / or RESPONSIBILITIES include the following :
- Perform telephone and / or virtual triage to assess patient symptoms using clinical guidelines and protocols.
- Provide safe, effective, and appropriate advice or interventions based on clinical assessments.
- Document all patient interactions accurately and thoroughly in the Electronic Health Record.
- Identify potentially emergent or life-threatening conditions and respond according to clinical protocols (e.g., directing to ER, activating EMS).
- Collaborate with physicians, advanced practice providers, and other healthcare professionals to coordinate care.
- Educate patients on self-care measures, medication use, and when to seek in-person care.
- Participate in quality assurance activities to ensure adherence to clinical standards and protocols.
- Support patient scheduling and referrals based on triage outcomes.
- Manages incoming and outgoing mail and faxes.
- Prepares and distributes correspondence, reports, and other documents as needed.
- Assists with data entry and maintenance of various clinic databases.
- Facilitates communication between patients, medical staff, nursing staff, and other departments within the clinic.
- Relays messages and information accurately and in a timely manner.
- Processes and relays important information to providers, nurses and other healthcare staff.
- Schedule follow up appointments for the patients.
- Maintain confidentiality and comply with HIPAA regulations, tribal policies, and other applicable laws.
- Assists with special projects and initiatives as assigned by the Clinical Providers or Director of Nursing.
- Other duties as assigned.

EDUCATION AND EXPERIENCE :
Registered Nurse (RN or LPN) with an active license.Minimum 2 years of clinical nursing experience (emergency, primary care, or telephone triage preferred).CERTIFICATES and / or LICENSES :
Current Basic Life Support (BLS) certification.Must possess and maintain a valid driver's license and eligible for tribal insuranceMust pass a background check and pre-employment drug screen.PLEASE NOTE : P.L. 93-638, Indian Preference and Tribal Employment rights Ordinance.
