About the Role
In this position, you will conduct comprehensive health assessments during visits that can last up to one hour, reviewing members' health status, medical histories, and more. This role allows you to gather critical insights that are often missed in standard office visits. Responsibilities include :
- Conducting detailed adult and geriatric health assessments, including diagnosis, treatment, health education, and referrals
- Administering and ordering necessary point-of-care and lab testing
- Collaborating with physicians, case managers, and family members to coordinate comprehensive care
- Providing services at the member's residence, including homes or Skilled Nursing Facilities

Master's Degree or equivalent experience, along with NP or PA licensureCurrent NP or PA license in state of practice with prescriptive authorityBoard certification in a relevant specialtyCurrent BLS, ACLS, or CPR certificationAt least 1 year of experience as a Nurse Practitioner or Physician Assistant preferred (new graduates are encouraged to apply!)Valid driver's license and sufficient automobile insuranceStrong computer skills and proficiency in medical record softwareExcellent verbal and written communication skillsThe Matrix Culture
