Job description

Qualifications : Required :

  • Master's degree or higher
  • Over 1 year of Healthcare Setting experience
  • Valid Physical Therapist in the State of Illinois (IL)
  • Valid Regular Driver License (DL)

Overview :

You will be responsible for providing Physical Therapy services to patients in their homes, assessing patient functioning levels, and developing treatment programs to improve independence and physical / cognitive functioning. You will also be involved in patient and caregiver education, supervision, and reporting on patient progress.

Key Responsibilities :

  • Perform Physical Therapy assessments and develop treatment programs
  • Record and report the patient's reaction to treatment and condition changes
  • Instruct patients and caregivers in therapy programs and device use
  • Supervise physical therapy assistant and, when appropriate, the certified nursing assistant
  • Prepare reports and participate in staff and Agency development activities

    • Schedule and Shift Details :

  • Monday Through Friday
  • 8 : 00a / 9 : 00a-5 : 00p plus 1 weekend per month (Saturday & Sunday; day shift with scheduled patients)
  • 2 holidays (1 summer / 1 winter)
  • No on-call

    • Travel :

  • Local travel to residential homes
