Job Summary :

As the Pediatric Primary Care Social Worker, you provide care management for high-risk pediatric patients and family members / caregivers managing psychosocially complex conditions.

You will provide social work and care management services to pediatric patients and caregivers to promote understanding and resolution of problems.

REQUIREMENTS

• Education : Master's Degree in Social Work
• Skills : Exceptional problem-solving, critical thinking, organizational, written / verbal communication skills, and high computer literacy. Ability to work in a self-directed environment with attention to detail and follow-through.
• License : Active LISW license
