Position Summary :

We are seeking a dynamic, compassionate, and experienced Hospice Clinical Liaison to join our team. This is a hybrid position that includes clinical and business development and will report to the Director of Business Development. The Clinical Liaison serves as a vital bridge between hospitals, physicians, referral sources, and our hospice services. This role involves educating patients, families, and healthcare providers about hospice care options while evaluating and coordinating appropriate hospice admissions.

Key Responsibilities :

  • Serve as the primary point of contact for hospitals, physicians, and other referral sources.
  • Educate patients and families on hospice services, goals of care, and eligibility requirements.
  • Conduct clinical assessments to determine hospice appropriateness.
  • Coordinate timely admissions by working closely with the clinical and admissions team.
  • Maintain strong relationships with referral sources to promote understanding and utilization of hospice services.
  • Collaborate with interdisciplinary team members to ensure smooth transitions of care.

Education :

  • Graduate of an accredited School of Nursing.
  • Licensure / Certification :

  • Valid Texas nursing license.
  • Current Driver’s License.
  • Valid BLS.
  • CHPN preferred.
  • Experience :

  • Previous experience in hospice care, healthcare marketing, or healthcare liaison roles.
  • Excellent verbal and written communication skills.
  • Skills :

  • Excellent communication and interpersonal skills.
  • Ability to build and maintain relationships with a diverse range of professionals.
  • Strong understanding of the Medicare Hospice Benefit eligibility criteria.
  • Proven ability to deliver engaging presentations and interact with both small and large groups.
  • Demonstrates they are self-motivated and able to work independently while collaborating effectively within a multidisciplinary team.
  • Strong clinical assessment skills.
  • Proficiency in Microsoft Office and CRM software for maintaining accurate records.
  • Schedule :

  • 8 hour shift.
  • Day shift.
  • Monday to Friday.
  • On call rotation.
  • Ability to Commute :
