Responsibilities :

• Work closely with AWARE's Clinical Professionals in providing services as assigned and developed through prescribed curriculum lesson plans.
• Spend time with client and family as assigned and / or as identified on the treatment plan.
• Assesses client, family and environments utilizing assessment tools.
• Coordinates and conducts the team process to develop a comprehensive Individualized Treatment Plan.
• Provide services to identified children and families in order to assist in gaining access to :
• Medical Services - Social Services - Educational Service - Vocational Services - Recreational Services
