• Minimum 1 year Acute care experience required.
• Organize and implement individual patient care based on ongoing interpretation of physiological, psychological, sociological deficits affecting the state of health
• Case management of patient care, and admissions
• Coordinate and educate patient and family as needed regarding care
• Evaluate patient care and progress
• Identify areas of support for family and patient needs
