JobSummary : Provide nursing care to patient in the home setting. Observe and assess the client and / orcaregiver to enhance the quality of life. Demonstrate individualized creativity in educating the patient and / orcaregiver. Follow nursing policy andprocedure per agency standards. Followthe plan of care according to physician orders. Demonstrate understanding of Medicare and Medicaid regulations.
Qualifications / Educational Requirements :
1. LicensedPractical Nurse.
2. Minimumof 2 years experience in a healthcare setting.
3. Excellentoral and communication skills.
4. Bi-lingual preferred (Portuguese, CapeVerdean Creole and Spanish)
Responsibilities / essentialfunctions : The person in this position must be able to perform the following essential jobfunctions with or without reasonable accommodations.

4. Performnursing procedures according to agency policy and procedures.
5. Workunder the direction of a RN.
6. Monitorreactions and patient progress using observation, assessment, and evaluationskills.
7. Educatepatients and family members according to disease process, medications, POC, treatment options, and homecare procedures according to the plan of care.
8. Reportto physician and RN of adverse findings.
9. FollowMedicare and Medicaid regulations.
10. Coordinateand monitor patient care and services.
11. Complywith HIPAA regulations in and out of the office.
12. Followinfection control policy in and out of the office.
13. Documentskilled visit according to Medicare guidelines.
14. Maintainpatient records according to policy and procedures.
15. Participatein in-services, workshops, seminars, and self-study courses annually.

Employee works in an office environment sometimes with moderate noise levels,controlled temperature conditions and sometimes travels to patients homes wherethey may have direct exposure to hazardous substances. Employees may interactwith upset staff and / or public and private representatives in interpreting andenforcing departmental policies and procedures.
Employee will have to travel to a variety of patient homes and perform inconditions that vary greatly depending upon the client's home environment. Somehomes will be clean, neat, and maintained at a comfortable temperature. Otherhomes may be cluttered, dirty, with an uncomfortable temperature.
