Job Title : Physician Navigator / Practice Regional Lead

Location : Carmel, Indiana

Job Type : Full-Time

Job Schedule : 8a to 6p weekdays, flexibility required

Compensation : $26 to $28 per hour

Benefits : Medical, Dental, Vision

Work Settings : Office, Start-up (Not a remote position)

Responsibilities :

• Patient Navigation and Care Coordination :

Serve as the point person and subject matter expert for patients and key channel partners, guiding them through the Posterity Health service offerings, value propositions, and next steps.

Be a leader on the clinic teams in problem-solving to address issues related to patient safety, care quality, and service excellence.
