Position Summary
The Registered Dietitian (RD) is responsible for providing nutrition counseling and education to patients with identified nutritional risk factors, especially those with obesity and diabetes . As a Diabetes Care and Education Specialist , the RD leads individual and group sessions, manages a patient panel, and collaborates with a multidisciplinary care team to promote wellness through a holistic, faith-integrated approach.
Essential Responsibilities 1️⃣ Individual Counseling
Conduct initial assessments and follow-ups using the EMR (Centricity)
Set and document personalized, attainable nutrition goals
Provide patients with goal summaries and relevant educational tools
Collaborate with providers to create appropriate treatment plans
Assess and refer patients for psychological, social, and spiritual needs
Practice cost-effective, patient-centered care
2️⃣ Group Education
Lead group nutrition education sessions with patients or community groups
Create or adapt lesson plans and educational materials
Clearly communicate your role and clinical goals during group sessions
3️⃣ Diabetes Care & Education
Participate in "Diabetes Day" clinics, offering instruction on :
Glucose monitoring (finger-stick & CGM)
Insulin and GLP-1 injections
Insulin titration protocols
Coordinate monthly group medical visits
Track and report diabetes metrics and productivity
Collaborate with CHWs to enhance diabetic patient care
4️⃣ Community Health Engagement
Promote healthy living at health fairs, cooking demos, and community classes
Prepare and present culturally appropriate health materials
5️⃣ Prenatal & Breastfeeding Nutrition
Partner with OB team to provide breastfeeding education
Counsel prenatal and postpartum patients on maternal and infant nutrition
6️⃣ Team Communication & Professional Development
Collaborate regularly with staff and medical supervisors
Respond promptly to urgent patient or team needs
Stay up to date with continuing education
Proactively suggest and support practice improvements
Knowledge, Skills & Competencies
Bachelor’s degree in Food & Nutrition or Dietetics from an accredited program
Completion of approved internship and CDR registration (or within 6 months of hire)
Eligibility for PA Licensure as a Licensed Dietitian-Nutritionist (LDN)
Bilingual (Spanish / English) required
Strong communication, organization, and interpersonal skills
Intermediate computer skills and proficiency in Microsoft Office
Ability to work independently and maintain confidentiality
Passion for integrating Christian faith into patient care
Working Conditions
Frequent use of computers and electronic medical records
Occasional lifting and moving up to 40 lbs; physical flexibility required
Requires mobility within clinical and community settings
