Position Overview

All HomeCare Services is seeking a licensed and motivated Physical Therapist (PT) to provide in-home therapy services in the Niles, Michigan area. This is a flexible position ideal for therapists who enjoy autonomy and creating their own schedule, provided patient visits are completed within the required timeframe.

Key Responsibilities :

• Conduct in-home physical therapy assessments and develop individualized treatment plans
• Deliver evidence-based therapy interventions tailored to patient needs
• Educate patients and caregivers on exercises and therapeutic techniques
• Document all visits and progress in compliance with agency policies and state regulations
• Collaborate with other healthcare professionals to ensure coordinated patient care
