Licensed Practical Nurse - Full Time Evenings

Position Overview

Full Time Evenings, 5x8 per week.

Every other weekend and holiday coverage required

Opportunity to provide direct and indirect skilled nursing care for assigned residents

Work under supervision of Registered Nurse following defined policies and procedures

Essential Responsibilities

Provide care appropriate to the age of patients served on assigned unit

Demonstrate knowledge of growth and development principles across the life span

Assess data reflective of patient's status and interpret appropriate information

Identify each patient's requirements relative to age-specific needs

Provide care needed by adult and geriatric patient groups

Maintain collaborative relationships with nursing staff, physicians, and ancillary staff

Maintain appropriate level of knowledge and clinical expertise through continuing education and in-service programs

Key Job Duties

Collect data relevant to resident's physical and psychosocial condition on ongoing basis and communicate findings to RN and other members of the Interdisciplinary Team

Contribute to creation of developmentally age appropriate and culturally relevant plan of resident care reflecting nursing diagnosis and interventions to achieve desired resident outcomes involving the resident / family and healthcare team in this process while promoting a healthy, safe environment

Recommend revisions to the plan of care, after evaluation of resident outcomes and indicate the need for referrals to ensure all aspects of the resident's care have been addressed

Collaborate with the resident / family and members of the Interdisciplinary Team within the scope of nursing practice assuring customer satisfaction

Maintain current knowledge and competency in Nursing Practice

Participate in the mentoring / orientation of staff

Consider factors related to safety, cost, and impact in planning in delivery of care

Under the direction of the RN, provide leadership in the professional practice setting

Demonstrate knowledge and competency

Required Qualifications

Current NY LPN Licensure

BLS certification through AHA or RQI specifically

Minimum 1 year experience as LPN

1 year experience serving in an LPN capacity at LTC Facility

Important Information

Occupational exposure to blood borne pathogens is reasonably anticipated

Required tasks involve potential contact with blood, body fluids, or tissues
