Position Summary : The DON is responsible for the coordination of health care needs, evaluation and improvement of program services and the delivery of high-quality services through the nursing process. Ensure quality services consistent with the philosophy of the Assisted Living Community and comply with applicable state laws and regulations. Conduct all responsibilities within the Scope of Practice dictated by the state. Meet Federal and State regulations, as applicable. Provide personal care services, assistance, support, evaluation, and supervision to residents, as needed. Candidates will have the ability to enrich the resident's lives by addressing their physical, intellectual, emotional and social well-being.

Essential Job Responsibilities :
Coordinate services to meet the health needs of the residents and promote wellness.Provide personal care services, assistance, support, evaluation, and supervision to residents, as neededPerform pre-admission assessments of potential new residents.Meet with every new resident and complete basic health assessment within one week of their moving into the community and update service plan within 30 days.Complete 90-day medication reviews (POS-Physician Order Sheets) and 30-day health evaluation reviews for all residents.Develop an assistance plan for each new resident.Work with team to develop and update service plans for residents.For at-risk residents, coordinate the assistance plan, activate resources through home health, and communicate with the physician and responsible party regarding issues that keep the resident at risk.Ensure the overall accountability for specialty units such as Dementia / Alzheimer's units.Based on physician orders, guide, evaluate, perform and supervise resident assistance plans including but not limited to, assessments, resident / family teaching, development, and updating of residents' plans of assistance Ensure the implementation of the Resident Service Plan and evaluation of that plan to include updates for significant changes.Establish and maintain resident records according to State and Federal guidelines.Review incident reports for completeness and follow-up documentation in the medical records.Provide or coordinate educational opportunities related to health issues of interest to the residents and families (i.e. Alzheimer's support group).Establish a medication setup system that works for each resident so that residents can remain independent in taking medications.Communicate changes in the resident's overall condition to the responsible party, physician, Executive Director, and other team members.Assume on-call responsibilities and address medical emergencies as they arise.Responsible for the Quality Improvement, Safety, and OSHA programs for the nursing department.Education, Experience, and Other Requirements
