A company is looking for a Care Coach II to support family caregivers and their care recipients. Key Responsibilities Engage and educate caregivers via phone and digital technology to improve their wellbeing Manage a caseload and assess caregiver and care recipient needs, assisting in care plan goals Document coaching activities and collaborate with stakeholders to identify problems and solutions Required Qualifications and Education 4+ years of experience in care planning or case management Experience working with the elderly or adults with disabilities Knowledge of industry concepts and principles related to coaching support Proven experience in accessing community resources Ability to navigate and teach the use of mobile applications
