Administers comprehensive physical therapy modalities in the home setting. Assists the physician in evaluating level of function. Helps develop the plan of care and revises it as necessary. Prepares clinical and progress notes. Advises and consults with the family and other agency personnel. Participates in in-service programs. Maintains effective working relationships as part of the team to meet goals and objectives. Utilizes resources, supplies, and time efficiently to contain costs. Promotes positive, respectful communication to all external sources. Displays behaviors that support the organization as a whole. Supports public relations and marketing initiatives within the community. Strives for continuous improvement in all aspects of the Physical Therapist function. Develops and revises treatment plans in collaboration with patients, families, physicians, and team members. Provides an environment that respects patients’ privacy and confidentiality. Initiates appropriate rehabilitative programs. Supervises Physical Therapist Assistants to ensure patient safety and compliance with the plan of care. Consults with and educates patients and families regarding disease processes and self-care techniques. Implements home therapy programs including gait training, transfers, and activities of daily living. Periodically evaluates patient progress and adjusts therapy programs as necessary. Directs, coordinates, evaluates, and supervises the quality of patient care services. Interacts with patients, physicians, and referral sources in a manner conducive to positive relationships. Displays patience, courtesy, and tact effectively. Supports case management designed to meet individual health and service needs. Participates in patient conferences to ensure comprehensive coordinated plans of treatment. Assists in the orientation of new rehabilitation personnel. Ensures completion of all appropriate clinical records for compliance with legislation. Refers to appropriate community agencies and maintains knowledge of available services. Demonstrates flexibility and a willingness to cooperate while working with others. Ensures problem resolution at the point of contact. Makes constructive recommendations for improving patient care and external customer service. Ensures that patient needs are met through the care delivery process. Participates in meeting operational and financial objectives. Monitors quality indicators, including customer satisfaction. Understands the impact of the Physical Therapist function on various outcomes. Pursues efforts to reduce or eliminate avoidable costs and errors. Ensures compliance with regulatory practices and guidelines. Responds to findings of quality management processes and resolves potential issues.
