To coordinate the care of our patients and assist with any Medicare issues.

Essential Duties & Responsibilities

• Review all admission documentation in order to ensure the quality of the services to be provided.
• Responsible for coordination with all field staff, that they follow agency's policies and procedures.
• Responsible for reviewing the patient's plan of care and ensuring that it gets properly implemented, including discussing it with the patients as necessary.
• Maintains daily communication with field staff in order to ensure that all services are provided as per patient's expectations.
• Responsible for reviewing all physicians' orders obtained by field staff.
• Verifies the proper delivery of daily services as scheduled, including unscheduled emergent visits in assigned community.
