Job descriptionOutpatient primary care and consultative practice located adjacent to Kent Hospital Patient population : geriatrics ages 65 appointments range from 30-60 minutes Scope of practice : primary care h&ps develop and implement treatment plans for a variety of conditions cognitive evaluations mobility management pre-operative geriatric assessment through our Geriatric Surgery Verification program Option for some time to be spent on our ACE inpatient unit at Kent Hospital Collaborate on system wide Age Friendly initiatives Minimum 1 year experience Only Geriatric ED Accredited hospital in RI (ACEP Silver Level 2) One of 43 national ACE Units IHI Committed to Excellence approved One of 25 hospitals in the nation with Geriatric Surgery Verification designation Innovative Hospital at Home program Robust academic affiliation with Brown University and University of New England College of Medicine EPIC integrated system wide
Kent Hospital a 360 bed teaching hospital part of Care New England a top non-profit health system in RI and southeastern MA is seeking an Advanced Practice Caregiver to join our dynamic Geriatrics and Palliative Care Team.
