Job Description :

The Corporate Director of Case Management is a key leadership position responsible for the assessment, planning, coordination, development, and direction of Case Management / Transitional Care, Post-Acute Services support, Social Work programs, and Case Management-related Regulatory Standards. This role extends to initiatives in areas such as automation and information systems. The Director will collaborate with cross-functional teams to implement comprehensive programs, identify improvement opportunities, and standardize processes to enhance outcomes and organizational efficiency.
