seeking a compassionate and driven Physician to build and manage a
dedicated panel of patients across independent and assisted living
facilities. This is more than a clinical role—it’s an opportunity
to establish lasting relationships with residents, champion
preventive care, and directly impact the health, dignity, and

relationships by developing a dedicated panel and providing
continuity of care. Direct Patient Care : Conduct assessments,
manage chronic conditions, and deliver preventive services as the
primary physician for residents. Routine Rounds : Perform regular
visits at assigned facilities, addressing both ongoing and acute
health needs. Care Coordination : Partner with facility staff,
families, and interdisciplinary teams to create personalized,
holistic care plans. Documentation & Compliance : Ensure
accurate, timely EMR documentation while adhering to all regulatory
standards. Provide education on preventive health, chronic
condition management, and wellness strategies. Quality Improvement
: Participate in initiatives that drive better outcomes and elevate
