We are seeking a compassionate and skilled Wound Care Nurse Practitioner to join our team providing specialized wound care services to patients in nursing home facilities across Central California. The ideal candidate will possess strong clinical skills, excellent communication abilities, and a passion for geriatric care.
Available Positions
- Full-Time Position : 5 days per week (40 hours)
- Part-Time Position : 3 days per week (24 hours)
Key Responsibilities
Conduct comprehensive wound assessments and develop treatment plans for nursing home residentsPerform wound debridement, dressing changes, and other wound care proceduresDocument patient conditions, treatments, and progress according to professional standardsCollaborate with interdisciplinary healthcare teams to optimize patient outcomesEducate nursing staff and caregivers on wound prevention and managementFollow up on patient progress and adjust treatment plans as neededTravel to different nursing home facilities (one facility per day)Manage a daily caseload of 20-30 patientsQualifications
Current Nurse Practitioner license in CaliforniaMaster's degree or higher in NursingAt least one year of clinical experience as a Nurse PractitionerExperience in wound care preferred but not required (no wound care certification necessary)Excellent assessment, critical thinking, and decision-making skillsStrong interpersonal and communication abilitiesValid driver's license and reliable transportationWork Schedule
