Reports to the Nurse Manager / Director and is responsible for coordinating and delivering patient care utilizing the nursing process in a hospital setting. Participates in identifying and meeting learning needs of self. Attends education programs based on identified learning needs. Uses patient education materials relevant to the patient population with assistance. Assesses readiness to learn of the patient, family, and caregivers. Evaluates expected patient outcomes and seeks guidance for care plan revisions as needed. Participates in discussions with members of the interdisciplinary team regarding patient care evaluation. Communicates relevant information to promote continuity of care. Demonstrates competence in caring for patients with complex problems, addressing physical, psychosocial, educational, and safety aspects of care. Performs bedside point of care testing as required for patient care plans. Utilizes organizational and nursing standards, policies, and procedures in care delivery. Organizes and prioritizes care according to patient and family needs. Consults with appropriate resources regarding patients with complex care issues and high-risk discharge planning. Develops goals to promote professional growth with assistance. Achieves goals and objectives within identified time frames or renegotiates with supervisor. Is knowledgeable about activities that facilitate collaboration between departments. Participates in development and achievement of unit goals and performance improvement activities. Effectively uses communication systems. Promotes cost-effective care and provides feedback to co-workers. Is aware of public policy and regulatory guidelines affecting the healthcare environment. Promotes a safe, clean, and secure environment for all. Recognizes data from complex situations to determine priorities for care. Synthesizes assessment data into meaningful information before communicating to others. Assesses and anticipates discharge needs of patients and families. Develops relationships with families to empower their ability to advocate for the patient and their needs. Collaborates with patients and families to prepare or update the plan of care. Utilizes multidisciplinary resources in planning care. Addresses cultural and ethnic issues in patient care planning. Demonstrates ability to prioritize tasks for patients with complex problems.
