Job descriptionAssess clients strength mobility gait range of motion and pain to create tailored treatment plans. Communicate changes in condition or treatment response to the Director of Nursing and physician. Implement therapy programs and educate family members and caregivers on how to support care. Prepare clinical documentation progress summaries and participate in discharge planning. Provide respectful professional care to clients families and staff. Degree in physical therapy from an accredited program. Successful completion of the NPTE and / or state exam. Active state licensure. Minimum of one (1) year of physical therapy experience. Ability to maintain HIPAA compliance and protect client confidentiality.
