Case Manager

Location : Alameda, CA

Work Hours : Monday to Friday, 9 AM to 5 PM

Pay Rate : $100K-$120K per year

We're looking for a Case Manager to join our in-office team, supporting clients who need help organizing services and staying on track. This role involves building one-on-one relationships, staying organized, and making sure referrals and resources actually turn into results.

Day-to-Day Responsibilities :

  • Work with clients to create manageable, realistic care plans
  • Coordinate services, referrals, and appointments
  • Maintain accurate and up-to-date files
  • Follow up with clients and providers regularly
  • Adjust plans as needs shift over time

Requirements

  • Bachelor's degree in social work, nursing, psychology, or similar field
  • 2+ years of experience in case or care coordination
  • Strong organizational and record-keeping abilities
  • Working knowledge of local service systems
  • LCSW, LMSW, or RN license is a plus
