JOB SUMMARY

The Infectious Disease Service Integration and Navigation Coordinator will support the Viral Hepatitis Program Manager in improving access to hepatitis and substance use services through, training, education, and other outreach to appropriate stakeholders in Philadelphia Primary responsibilities include supervising HCV navigators; assisting utilization of key partners for a Technical Advisory Committee; curating, maintaining relationships with key stakeholders; compiling reported information to assist in the formation of a Provider Toolkit; and coordinating training and technical assistance for partner sites. This position is focused on building clinical capacity to test and treat more people living with HIV, hepatitis B, and hepatitis C. The person in this role will coordinate and provide guidance to the hepatitis C / substance use navigators, in collaboration within the Program and within the Health Department. This person will further collaborate on activities supporting the viral hepatitis elimination goals of the Viral Hepatitis Program as a whole.

JOB SPECIFICATIONS

Responsibilities / Duties

  • Supervise staff conducting case management and navigation work for people living with hepatitis B and / or hepatitis C.
  • Monitor data and case completion for all navigation activities.
  • Coordinate navigation programming activities, in collaboration with the Substance Use Prevention and Harm Reduction Division (SUPHR).
  • Provide technical assistance and guidance to other members of the team, supporting group problem solving.
  • Develop and update protocols for outreach and navigation, with support of Program Manager, as necessary.
  • Support data collection and management for programming and public health surveillance needs.
  • Recruit and support Technical Advisory Committee members with expertise in people who use drugs (PWUD), harm reduction, and infectious diseases associated with drug use.
  • Organize, plan, execute, facilitate Technical Advisory Committee meetings.
  • Work closely with an established hepatitis program to develop a Provider Toolkit for service integration among locations caring for PWUD.
  • Review and interview national, state, and local resources and stakeholders, respectively, to integrate models of care into the Provider Toolkit.
  • Promote integration of viral hepatitis prevention, vaccination and testing programs into organizations treating PWUD.
  • Coordinate cross-divisional collaboration within PDPH.
  • Assist in community engagement initiatives and activities, e.g. social media, websites, events, education.
  • Participate in viral hepatitis elimination planning and implementation activities.
  • Assist with reports to funding organizations or for grant writing.
  • Build and maintain relationships with potential partner sites.
  • Support and mediate data reporting from partner sites to PDPH surveillance programs, including Hepatitis and SUPHR teams.
  • Execute an evaluation plan to track project implementation and management.
  • Coordinate training and TA needs for partner sites to improve prevention, testing, and treatment for HBV, HCV, HIV and other infectious diseases associated with drug use.
  • Update and / or develop educational materials to disseminate at partner sites and more broadly.
  • Develop and provide training and education for viral hepatitis targeted to at-risk populations and to providers across specialties.
  • Participate in local, state and national conferences, conference calls and workgroups addressing infectious disease consequences of drug use and related public health topics.
  • Support broad program activities through close work with the Viral Hepatitis and SUPHR teams.
  • Participate in emergency preparedness exercises and real response activities at the discretion of the Viral Hepatitis Manager and Division Director.

Education

  • Master's degree in Social Work, or similar field from an accredited college or university with two(2) years of related experience. OR
  • Bachelor's degree in public health or similar field plus four (4) years of relevant experience.
  • Skills / Experience

  • Performing community outreach, preferably about health or social services programs.
  • Working with individuals with opioid use disorder, preferred
  • Working in a health care setting (health department, hospital, FQHC, primary care, community-based organization, etc.)
  • Able to maintain confidential medical information.
  • Knowledge of and ability to work and engage with individuals who are part of specific populations, including incarcerated individuals, persons who use drugs, recent immigrants, uninsured, under-served and under-represented populations.
  • Knowledge of harm reduction, stages of change, and motivational interviewing, preferred.
  • Working knowledge of viral hepatitis, including transmission, prevention strategies, laboratory and treatment information preferred (additional training will be provided).
  • Extensive experience engaging with diverse community members, including public speaking, training, and / or individual and small group engagement.
  • Strong written and verbal communications skills.
  • Capacity to manage data collection and management.
  • Able to handle multiple tasks and support project timeline with a high degree of organizational skill.
  • Proven experience in meeting facilitation and recruitment.
  • Must be skilled in Microsoft Office suite.

  • Able to work effectively and professionally in a fast-paced environment.
  • Fluency in Spanish a plus.
  • Work Environment : Standard office setting with extended periods at workstation and use of office equipment. This position also requires extensive time in the field interacting with and linking clients to care.

    Position Type and Work Schedule : Full time position, typical hours are Monday through Friday 8 : 30 am to 5 : 00 pm, with occasional need for work outside of typical hours and on weekends. Work location is the Philadelphia Department of Health Division of Disease Control office at 1101 Market St. 12th Floor in Philadelphia.

    Travel : Local travel to multiple sites several times per week, as needed.

    Physical Demands : Ability to transport materials on occasion; walking to meetings in nearby offices.
