As our MDS Coordinator (RNAC), you will oversee the Minimum Data Set (MDS) assessment process with a mix of on-site and work-from-home components. You’ll ensure accurate, timely assessments that support high-quality resident care and optimal reimbursement.
What You’ll Do
Oversee and complete MDS 3.0 assessments accurately and on time
Collaborate with the care team to develop, review, and implement individualized care plans
Ensure full compliance with NYS and CMS MDS regulations
Monitor reimbursement accuracy and support quality improvement initiatives
Leverage hybrid schedule—conduct assessments on-site and perform related administrative tasks from home
What You’ll Need
Active RN license in New York State
Proven experience as an MDS Coordinator in skilled nursing or long-term care
Strong attention to detail, organizational skills, and familiarity with MDS 3.0 and PDPM
Effective communication skills to engage with staff and leadership
Ability to manage a hybrid schedule with both clinical and desk-based responsibilities
Compensation & Benefits
Annual salary range : $90,000–$100,000
