Job Title : Level 2 Registered Nurse Case Manager Profession : Registered Nurse Specialty : Case Manager Duration : 13 weeks Shift : Monday to Friday Hours per Shift : 8 hours (8 : 00 am - 4 : 30 pm) Experience : Minimum one year of experience as a clinical nurse in a hospital required; 9 months of home care experience preferred License : Current Client RN License required Certifications : BLS certification required Must-Have : - Certified Home Health Care experience required - OASIS experience required - Telemetry experience required - EPIC or Netsmart preferred Description : Develops and revises plans of treatment and discharge in collaboration with patients, families, physicians, and care delivery teams. Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition according to defined time frames. Processes orders and notifies physicians of patient needs and changes in condition. Completes certification, recertification orders, and discharge summaries. Provides an environment that promotes respect for patients, their privacy, confidentiality, and property. Initiates appropriate preventive and rehabilitative strategies. Provides services requiring substantial specialized nursing skills. Consults with and educates patients, families, and team members regarding disease processes, self-care techniques, and preventive strategies. Supervises LPNs to ensure patient health, safety, and compliance with the plan of care. Directs, coordinates, evaluates, and supervises the quality of patient care services provided by the agency. Interacts with patients, physicians, referral sources, and others in a manner that maintains positive relationships. Supports care management philosophy, focusing on customer-focused planning and case management. Ensures compliance with state and federal legislation and delivery of seamless services within established timeframes. Maintains current knowledge of available community-based services. Demonstrates flexibility and willingness to visit patients based on need. Ensures problem resolution at the point of contact. Attends patient care conferences regarding assigned patients. Participates in attaining operational and financial objectives of the agency. Monitors results and takes corrective action on quality indicators, including customer satisfaction. Understands the impact of RNs on patients and care providers and works to reduce avoidable costs and errors. Responds to findings of Quality Management reviews and ensures resolution of potential problem areas. Participates in peer review and performance improvement activities as assigned. Maintains productivity requirements. Ensures appropriate visit authorizations are in place and that documentation reflects medical necessity for home care.
