Clinical Care Coordinator

A company is looking for a Clinical Care Coordinator.

Key Responsibilities :

Ensure timely appointments and scheduling for members

Assist with the implementation and facilitation of services for members

Monitor compliance on scheduled services and document care accurately

Required Qualifications :

High School Diploma / GED required

Preferred healthcare industry experience

Minimum of one (1) year experience in home care, discharge planning, or case management

Preferably experience with the elderly frail population

Minimum of one (1) year experience in the managed care industry is preferred
