A company is looking for a Health Coach (Remote). Key Responsibilities Conduct assessments of members' health status to improve medication adherence and health outcomes Utilize motivational interviewing to engage members and promote behavior changes for optimal health Provide education and support to empower members in their healthcare decisions and integrate them into available programs Required Qualifications 1-3 years of experience with Microsoft Office 3-5 years of practice experience in relevant fields such as Renal, Diabetes, or Primary Care Experience in Managed Care and Medicare Must reside in EST / CST time zones Hardwired internet connection required
