Compensation
The pay for this position is $34 per hour .
Benefits are available to full-time employees after 90 days of employment.
A 401(k) with a company match is available to full-time employees with 1 year of service on our eligibility dates.
Highlights
- This position is a contract assignment with potential to hire permanently based on attendance, performance, and business needs.
- Work from Home position — must reside in Miami-Dade County, FL (Hialeah / Miami area) .
- Mileage reimbursable to and from member homes.
- Training will be conducted remotely via Microsoft Teams .
- Travel required approximately 75% of the time visiting members in their homes, assisted living facilities, or nursing homes.
Responsibilities
Conduct telephonic and face-to-face assessments to evaluate members’ medical and social needs.Develop individualized care plans addressing short- and long-term wellness goals.Coordinate care between primary care providers, specialists, and community resources.Manage prior authorizations, medication reviews, and condition management information.Provide education and support to promote healthy lifestyle choices and member engagement.Document all case management activities accurately and in compliance with company standards.Maintain regular communication with members, families, and healthcare professionals.Requirements
High School Diploma or GED required.Experience in case management, social work, or healthcare coordination preferred.Ability to multitask, prioritize, and adapt effectively in a fast-paced environment.Strong verbal and written communication skills.Tech-savvy and comfortable navigating multiple systems and software platforms.Self-motivated, detail-oriented, and organized.Attendance is mandatory for the first 90 days.Preferred Qualifications
