American Kidney Center of Louisville is looking for an enthusiastic, motivated, Renal Social worker to join our growing team!
The Renal Social Worker's responsibility is to help the patient and his / her family understand Chronic Kidney Disease (CKD) and End-Stage Renal Disease (ESRD) and renal replacement therapy, to counsel with the patient and family in working out their acceptance of the patient’s medical problem and methods of modifying ways for more satisfying relationships, to provide a psychosocial summary to aid in the preparation of the interdisciplinary patient plan of care, to act as a resource person and liaison with community agencies, to provide casework and group work services to assist the patient and family, to evaluate the patient’s optimum rehabilitation potential, to provide education regarding psychosocial problems of renal disease to members of the health care team and the public.
Job Responsibilities :
- Facilitate individual therapy for both home and in-center dialysis patients in an effort to guide them through their grief and adjustment to Chronic Kidney Disease (CKD), End-Stage Renal Disease (ESRD), and renal replacement therapy.
- Facility family counseling for AKC patients and their families as appropriate. Assist patients and families in understanding CKD and ESRD and the effects it has on quality of life.
- Includes MD, Dietician, and RN as an integral part of the treatment process. Assists patient and family in working effectively with entire treatment team.
- Completes in a timely fashion all documentation necessary to provide effective therapy, to meet agency standards, and to meet billing guidelines.
- Collaborates with treatment team in initial assessing and planning, as well as for ongoing monitoring and evaluation of the needs of the patient.
- Provides clinical input and recommendations to the treatment team. (Clinical input in this context is defined as follows : the professional application of social work theory and methods to the treatment and prevention of psychosocial dysfunction, disability, or impairment, including emotional and mental disorders. It is based on knowledge of one or more theories of human development within a psychosocial context. Clinical social work includes interventions directed to interpersonal interactions, intrapsychic dynamics, and life-support and management issues).
- Works with other members of the treatment team as needed to develop crisis intervention strategies and interventions for unstable patients.
- Participates in Quality Assessment Performance Improvement (QAPI) meetings on a monthly basis.
- Independently responsible for administering Kidney Disease Quality of Life Assessment (KDQOL) on every dialysis patient at least once a year and interpreting assessment using a clinical social work approach.
- Explore and develop family and community resources; maintains working relationship with various professionals, agencies, and volunteer organizations.
- Assists with applications for services and follow up referrals.
- Provides ongoing casework and group work services for patient and family.
- Assists in transplant evaluation and work-up.
- Assists in home training evaluation, work up, and follow up.
- Counsel with patients whose transplant has failed on grief and acceptance related to restarting dialysis treatment. Other duties and projects as assigned.
Minimum Requirements :
Social Work CSW in state of KY. Hold a masters degree in Social Work from a college / university accredited by the ASWB.1 year of healthcare experience preferred.
