Position Title : Care Manager (RN)

Work Location : Remote, FL (anywhere in FL)

Assignment Duration : 3 months

Work Schedule : 8 am - 5 pm

Work Arrangement : Remote, FL (anywhere in FL)

Position Summary :

• Enhanced Discharge Planning Team (EDCP), who make outbound calls to Medicaid health plan members who are discharged from the hospital to prevent re-admissions.

Candidate Requirements

Education / Certification

Required Associate or Baccalaureate degree in nursing

Preferred : Baccalaureate degree in nursing

Licensure

RN (compact is okay)

Preferred : RN

Must haves : 2-3 years Care Management experience with all age groups

Nice to haves : Discharge planning experience; Care Management Certification

Disqualifiers : Cannot answer questions regarding your past work experience during interview.

Performance indicators : Audit scores 90% or above monthly; 1 : 1 meetings with Supervisor

Best vs. average :

Must be good with technology

Outbound calling experience

Critical thinking

Must be able to work in fast paced environment; quick learner; hit the ground running for this temp role
