Self-starter, ready to begin immediately!!!Texas-licensed Physical Therapist.Home health experience, including OASIS and start of care knowledge.Understanding of Medicare, Medicaid, and private pay requirements (e.g., homebound rule).Must pass Texas DPS background check before onboarding, with annual rechecks, per Texas HHS standards.This position requires a valid driver’s license, proof of insurance, and a reliable personal automobile for local travel to provide care to clients in the Plano, Addison, and North Dallas Texas area. Mileage is reimbursed for start of care and regular visits.

Responsibilities :
Provide direct patient care to a diverse range of patientsComplete start of care and conduct thorough evaluations of patients to identify physical impairments and develop personalized treatment plans.Utilize a range of evidence-based techniques, including trigger point therapy, to address patient needs.Implement and monitor patient treatment plans, adjusting as necessary.Collaborate with healthcare professionals, including physicians and occupational therapists, to ensure comprehensive patient care.Stay current with the latest developments in physical therapy, including orthopedics, hospital medicine, and anatomy knowledge.Document patient progress and maintain accurate medical records.Educate patients on proper exercise techniques and provide guidance on maintaining a healthy lifestyle.Collaborate with other healthcare professionals to develop and implement individualized care plansProvide emotional support and educate patients and their families on disease processes and treatment optionsAssist in the development and implementation of policies and procedures related to patient careParticipate in quality improvement initiatives and contribute to the continuous improvement of patient careSkills :
Experience caring for patients of all agesKnowledge of geriatric and special needs populationsStrong would care knowledgeAbility to administer medications and perform catheter careFamiliarity with medical records and vital signsExperience with gastric lavage and other medical proceduresStrong communication and interpersonal skillsAbility to work effectively in a fast-paced environment and prioritize tasks efficientlyCommitment to providing high-quality patient care and contributing to the overall success of the organizationQualifications :
Bachelor's degree in Physical Therapy or Doctor of Physical Therapy (DPT) degree.Current licensure as a Physical Therapist in the State of TexasStrong knowledge of Oasis and experience writing start of careMinimum one year experience in a home healthcare settingOne-year experience in geriatric or pediatric care preferredExperience working in a hospital or clinic setting.Ability to provide patient care in a variety of settings, including home community care.Excellent communication and interpersonal skills.Proficiency in trigger point therapy and other physical therapy modalities.A passion for delivering high-quality patient care and a commitment to ongoing professional development.Qualicare - Come Join Us
